Important Information About Your Passport Application

The type of passport application you need depends on your situation. Please read details below carefully to ensure your process goes smoothly and avoids delays.

DS-11
  • For first-time applicants, minors, or lost/stolen passports

  • Requires an appointment at the post office

  • Must be sealed in an envelope by a postal clerk, then handed to Passport Noble

DS-82
  • For adult passport renewals

  • No post office appointment needed

  • Application can be handed directly to Passport Noble

  • Required if passport is lost or stolen

  • Must be submitted with DS-11 if applying for a replacement passport

  • This form provides legal notice that your previous passport is no longer in your possession and lost or stolen passport shall be invalidated

DS-64
DS-3053
  • For minors when one parent/guardian cannot appear in person

  • Must be completed by the absent parent/guardian

  • Must be notarized

  • Submitted at the post office with the DS-11 application

DS-86
  • Used when the passport is not delivered on time

  • Must be completed by an applicant or parent/guardian

  • Enables Passport Noble to retrieve the passport directly from the agency

  • Can be completed in advance as a precaution

  • This form is used when an applicant has lost a U.S. passport that is still active and unexpired, and has no other way to prove citizenship.

  • A file search verifies that a valid passport had been issued in the past. If approved, it will allow the applicant to move forward with their application.

  • Note: This option may only be used if the passport is still valid, has not expired, and has not been officially reported as lost or stolen.

DS-11

First-Time Passport

1. Application Form

Must be typed or written in black ink and easy to read. If form DS-64 form must be included be sure to do the same.

Do not sign the form until you're in front of a certified post office clerk.

Signature must match your legal name.

Passport Noble can assist with scheduling your post office appointment.

The sealed envelope from the post office is mandatory—it confirms your application is legitimate and complete.

If for any reason an applicant cannot sign due to a condition, a medical note is required.

2. Proof of Citizenship & Identification

Submit your original U.S. birth certificate or naturalization certificate.

Birth Certificate must include a file number and cannot be an abstract version.

Naturalization documents must show the official raised seal and USCIS Director's signature to be legible.

An active and valid identification must be provided over six months.

If you have no other means to prove your citizenship other than a lost passport that is still active, then a File Search Form can be submitted.

Minors/children

In regards to minors/children under the age of 16 years, both parents/guardians must be present at the post office with their active and valid identification of over six months. If the minor/child is at the age of 16 or 17, then one parent is permissible.

If a parent cannot be present with the minor/child, then a form known as "Statement of Consent" (DS-3053) must be filled out properly by the absent parent/guardian and notarized. Please note that the date of signature of the parent/guardian must be on the same date as the notary date, otherwise invalid.

A DS-3053 must be submitted along with a DS-11 (and a DS-64 if needed as well as a File Search Form if needed).

If a parent/guardian has abandoned the child, then the current parent/guardian must show legal documentation to have obtained full custody from the court.

A parent/guardian who may be incarcerated still needs to present a DS-3053.

3. Passport Photos (2x2 inches)

Must have a white background—no exceptions.

No glasses, no facial piercings, and ears must be visible.

Headgear is only allowed for religious reasons—with a written explanation.

Photos must be recent and clearly show your shoulders.

4. Letter of Authorization (L.O.A.)

This is not a generic form and must be provided by Passport Noble.

Do not proceed to the post office without first receiving the correct L.O.A.

The L.O.A. must be signed exactly as your passport form and included in the sealed envelope.

5. Travel Itinerary

A printed travel itinerary must show international travel within 14 days of submission.

If you do not have one, contact Passport Noble for assistance.

6. Payment: Check or Money Order

Payment amount may vary depending on processing needs.

Checks must be from the applicant (or parent/guardian for minors).

Money orders must be clean and clearly printed—faded ones will not be accepted.

DS-82

Passport Renewal

1. Application Form

Must be typed and printed or written in black ink and clearly legible.

Signature must match the one in your current passport book.

Updated signatures can be added to the new passport book once received.

If your name has changed, you must include documentation showing the legal name change.

Only 10-year passport books are eligible for renewal and passport books that have not surpassed over 5 years of expiration.

Exemptions apply for 2 year books issued under special circumstances.

Damaged or mutilated passports ------ including peeling, or unreadable pages, or missing pages ------ do not qualify for renewal.

If for any reason an applicant cannot sign due to a condition, a medical note is required.
